ORTHOPEDIC Q&A: Strength Training, Joint Pain & Staying Strong as You Age

As an orthopedic surgeon, I spend every day helping people recover from injuries, arthritis, and chronic pain. Some patients come to me after tearing a rotator cuff playing pickleball. Others have been living with knee pain for years before finally deciding it's time to ask for help. Many assume that pain simply comes with getting older.

What surprises most people is that the pain itself is often the last thing to appear.

By the time your shoulder hurts every time you reach overhead or your knee keeps you from enjoying a round of golf, the underlying changes have usually been happening for months or even years. That's why I often tell my patients one simple phrase:

Pain is a late signal.

In a recent conversation with my husband, orthopedic spine surgeon Dr. Jaspaul Gogia, we discussed the questions patients ask us every day about joint pain, strength training, back pain, imaging, surgery, and healthy aging. While our specialties are different, one message was remarkably consistent:

The best orthopedic care starts long before you ever need surgery.

Pain Is Usually the End of the Story, Not the Beginning

Many patients wait until they can no longer do the activities they love before making an appointment. Maybe tennis has become painful. Golf is no longer enjoyable. Walking the dog has become difficult.

By that point, inflammation, tendon damage, arthritis, or muscle weakness may have been developing for quite some time.

That doesn't mean every ache is an emergency, but it does mean you shouldn't ignore persistent symptoms. Small problems are often much easier to treat than large ones. Early evaluation may allow us to use physical therapy, activity modification, or other conservative treatments before more invasive procedures become necessary.

Listening to your body early can often change the entire course of recovery.

Movement Is One of the Best Medicines We Have

One of the biggest misconceptions I hear is that painful joints should simply be rested until they feel better.

In many orthopedic conditions, the opposite is true.

Movement helps lubricate joints, maintain muscle strength, preserve mobility, and reduce stiffness. Complete inactivity often causes muscles to weaken, flexibility to decline, and pain to become even worse.

That doesn't mean pushing through severe pain or ignoring an injury. It means finding the right kind of movement for your condition.

Physical therapy is frequently the first treatment I recommend because it helps patients continue moving safely while addressing the underlying problem.

Your Musculoskeletal Health Changes Long Before You Notice It

We spend a great deal of time talking about heart health and metabolic health, but we often overlook our muscles and bones until something breaks down.

Beginning in midlife, muscle mass naturally declines. Bone density also gradually decreases, especially in women after menopause.

The encouraging news is that these changes are not entirely out of our control.

Strength training helps preserve muscle, supports bone density, improves balance, and protects joints. Nutrition matters too. Adequate protein, vitamin D, calcium, and overall healthy eating patterns all play an important role in maintaining musculoskeletal health.

I like to think of muscle and bone as a retirement account. The more you invest while you're younger, the more you'll have available later in life.

Four Orthopedic Myths I Wish Would Disappear

Every week I hear the same misconceptions repeated in clinic. Here are four that deserve to be retired.

Myth 1: If It Hurts, Stop Moving

Rest certainly has its place after an injury, but complete inactivity is rarely the long-term solution.

Gentle movement helps maintain joint function and often reduces pain more effectively than prolonged rest.

Myth 2: Lifting Weights Is Bad for Your Joints

Many people, especially women, worry that resistance training will damage their joints or make them bulky.

The opposite is generally true.

Appropriately performed strength training builds muscle, supports joints, improves bone density, and reduces injury risk throughout life.

Myth 3: Walking Is Enough Exercise

Walking is fantastic. It improves cardiovascular health and is far better than remaining sedentary.

But walking alone is usually not enough.

Your body also needs resistance training, balance work, mobility exercises, and core strengthening to maintain long-term function and reduce injury risk.

Myth 4: An MRI Means You Need Surgery

One of the most common misconceptions is that any abnormal finding on an MRI automatically requires surgery.

In reality, many partial tendon tears, meniscus injuries, and degenerative findings can be successfully managed without an operation.

Images are only one piece of the puzzle. We treat people, not MRI reports.

What "Bone on Bone" Really Means

Patients frequently arrive saying they've been told they're "bone on bone."

Sometimes that's accurate.

Many times it isn't.

What people are usually describing is cartilage loss within a joint. Cartilage acts as a smooth protective surface that allows bones to glide over one another. As cartilage wears away with arthritis, the joint becomes less efficient and more painful.

Interestingly, imaging doesn't always predict symptoms.

Some patients have severe arthritis on X-rays and very little pain. Others have only mild arthritis but significant symptoms.

That's why good orthopedic care focuses on both the images and the person sitting in front of us.

Most Sports Injuries Don't Automatically Require Surgery

Rotator cuff tears, meniscus tears, tendon injuries, and many overuse conditions can sound frightening.

But the pain isn't always coming directly from the tear itself.

Often, inflammation surrounding the injured tissue contributes significantly to symptoms.

Reducing inflammation, restoring strength, and improving movement through physical therapy can dramatically improve function without surgery.

For some patients, regenerative treatments like platelet-rich plasma (PRP) may also be appropriate. Others may ultimately benefit from surgery, particularly with larger or complete tears.

The key is matching the treatment to the individual—not assuming surgery is the only option.

Back Pain Is Usually More Complex Than One MRI

Back pain is one of the most common reasons people seek orthopedic care.

When someone comes into my office with lower back pain but no significant nerve symptoms, we often start with the fundamentals.

Core strength.

Hip mobility.

Flexibility.

Weight management.

Physical therapy.

Many people underestimate how much weakness in the muscles surrounding the spine contributes to chronic pain.

One of the best investments you can make is spending a few minutes every day strengthening your core and improving mobility. Yoga, Pilates, and guided stretching programs can all play an important role alongside resistance training.

Osteoporosis Is Silent Until It Isn't

Unlike arthritis, osteoporosis often causes no symptoms until a fracture occurs.

That's why prevention is so important.

Weight-bearing exercise, resistance training, adequate calcium and vitamin D intake, avoiding smoking, limiting excessive alcohol, and appropriate bone density screening all help protect bone health as we age.

Strong bones don't happen by accident. They're built through years of consistent habits.

Weekend Warrior Injuries Are Real

One part of our conversation became especially personal when Dr. Gogia shared his own orthopedic injuries, including a lumbar disc herniation that required surgery and a quadriceps tendon rupture during a hike.

His experience reinforced something we see often in clinic.

Many active adults continue exercising into their forties, fifties, and beyond, but they prepare for workouts the same way they did in their twenties.

Our bodies change.

Warm-ups matter more.

Mobility matters more.

Recovery matters more.

Spending twenty minutes preparing your body may ultimately be more valuable than squeezing in one extra exercise.

Recovery Is Physical and Mental

An orthopedic injury doesn't just interrupt your workouts.

It can affect your identity, confidence, family life, and emotional well-being.

Many active people struggle with being forced to slow down.

Recovery requires patience, realistic expectations, and perspective.

The encouraging part is that most orthopedic injuries improve with appropriate treatment, rehabilitation, and time. While recovery isn't always quick, our goal is to help patients return to the activities that bring them joy.

Final Thoughts

After this conversation with Dr. Jaspaul Gogia, my biggest takeaway is simple:

Orthopedic health isn't just about fixing injuries.

It's about preventing them.

Build strength before you lose it. Protect your bones before they become fragile. Work on mobility before stiffness sets in. Don't wait until pain keeps you from living the life you enjoy.

Pain may be a late signal, but healthy habits can start today.

The strongest, most resilient bodies aren't built in the operating room. They're built through years of movement, strength training, recovery, and consistency.
